Data-Informed Transition Collaboration: A New Solution to Old Problems

This session introduces a data-informed model for collaboration among educators, VR counselors, and community providers working to improve outcomes for youth with disabilities. Presenters contrast this model with traditional approaches, highlighting the benefits of real-time coordination, shared outcomes, and actionable data. Using case examples and tools such as the Transition Planning Tool and Readiness Toolkit, the session explores how agencies can better align services, reduce duplication, and support meaningful employment and education goals throughout the transition process.
